We offer the following primary care services to adolescents over the age of 13, adults & the elderly. We do not accept pediatric patients at this time.
Chronic Care Management
If you choose our healthcare provider to be your primary care provider, you may need to ask about Chronic Care Management. This is a program that is initiated by you and your provider to help you manage chronic (long term) conditions and to try to prevent worsening of symptoms and hospitalizations.
Examples of chronic conditions include, but are not limited to, the following:
- Alzheimer’s disease and related dementia
- Arthritis (osteoarthritis and rheumatoid)
- Asthma
- Atrial fibrillation
- Autism spectrum disorders
- Cancer
- Cardiovascular Disease
- Chronic Obstructive Pulmonary Disease
- Depression
- Diabetes
- Hypertension
- Infectious diseases such as HIV/AIDS
Physical Exams
We provide sports/pre-participation physical exams for adolescents, teens and young adults.

- Minor Allergic Reaction

- Sinus Infection

Minor Injury:
- Laceration

Advanced Care Planning
Advance care planning gives everyone a chance to say what is important to them. It helps patients understand what the future might hold and to say what treatment they would and would not want. It helps people, their families and their healthcare teams plan for future and end of life care. This makes it much easier for families and healthcare providers to know what the person would want – particularly if they can no longer speak for themselves. You can make an appointment to discuss advanced care planning options. Your primary care provider always needs to have the legal documents in your chart that you choose to use.
